Registration will not be complete until all required sections have been filled out and submitted. Once your submission has been reviewed and approved, you will receive a confirmation email finalizing your child’s registration. Please note that your child’s registration is not finalized until you receive this confirmation email.
Note: Some policies, forms, or immunization requirements may change as a result of local, state, or federal regulations. You will be notified if any updates are required.

Payment is due at the time of registration. Prior to June 1st, those paying by credit card will be charged a 50% deposit. The remaining balance will be charged on the first business day in June and will be automatically billed to your credit card. For payment by check, cash or those registering after June 1st, payment in full is due with your registration. There is a $25 fee for returned checks.
This form is used to record your child’s summer camp registration and collect all required information in advance. We do not collect payment on this form. Registration cannot be completed and your spot is not held until we have received all of these required forms and payment from you.
To complete registration, families MUST have an active account on file with a payment method saved. New families can create an account here; active/returning families can log in here. Once your account is set up, you may submit this form. This form will provide an estimated camp tuition amount; however, no payment will be processed upon submission of this form. Please note that this amount is an estimate only, as discounts or adjustments may be applied internally at a later time.

Please provide a list of individuals who have permission to pick up your child at the end of the camp day. The parent(s)/guardian(s) are automatically added as an authorized pickup person. You may add up to 5 total authorized persons.
Please upload a health report for each camper. The report must be dated within the last 12 months. If the report is older than 12 months, the registration will be incomplete and cannot be accepted or processed.

All children and any individuals seeking entry into the program space must be directed to pre-screen at home, prior to coming to the program for the day. Check for symptoms below. Anyone with a fever of 100.0°F or above or any other signs of illness must not be permitted to enter the program.
You have the right to request information about our written policies, including: discipline, background checks, health care, and grievance procedure. Energy Fitness & Gymnastics is in compliance for summer camps with the Massachusetts Board of Health and the Newton Health, Fire, and Inspectional Services Departments.
